I'm not sure, but it could also be option B. If Bob Smith doesn't have a delegate set up, that might explain why the approval is being skipped for him specifically.

Hmm, this seems like a tricky issue. I'm thinking it might be option C - the Bob Smith user account doesn't have a manager specified. That would definitely cause the approval to be skipped.
