Hmm, C and D sound a bit sketchy to me. You don't want to just erase all your hard work, and going back to the drawing board is a recipe for procrastination. A and B are the way to go.
I'm going with A and B. Deleting a risk seems risky, and setting it back to draft would be like starting over. Gotta keep that assessment moving forward!
A and B seem like the logical options here. You can't just delete a risk or set it back to draft - that would defeat the purpose of the assessment process.
