I don't know, the question seems a bit vague. Could it be that the management stakeholders aren't involved enough in the process? Maybe they need to be more hands-on.

Definitely B - the lack of cadence and synchronization between Product Management and the team is the root cause here. PI Planning is supposed to align everyone, not create chaos.
