Definitely B and D. Activating the workflow template and referencing existing notifications doesn't seem to have anything to do with enabling defects recording. Time to show off my configuration management skills!
Yeah, I agree. Defining the defect report type and assigning the notification type to the inspection type makes more sense for enabling defects recording.
