I'm not so sure. I think A and C might also be correct. The document can be assigned at operational level, and the header notification cannot be deleted.
I'm going with B and D as well. Anything that streamlines the process of creating maintenance orders gets a thumbs up from me. Now, if only I could get my car to fix itself...

Hmm, I'm not sure about A. Shouldn't we be able to assign the document to any level, not just the operational one? These maintenance order rules are getting a bit too complicated for my liking.
I agree with Jesusita. The ability to automatically copy the long text is super convenient. Although, I'm a bit surprised that the header notification can't be deleted - that seems like an oversight.
B and D seem to be the correct options here. Copying the long text of the reference notification into the order is a handy feature, and when creating an order from a single notification, that notification does become the header.
