I agree with Precious. A and B are the way to go. D is kind of obvious, isn't it? I mean, if the urgency hits 100, the user should definitely be notified.
A and B seem to be the correct choices here. Urgency value will continue to increment as configured, and it will remain at 100 while other service level processing continues.
