I'm not sure why the general manager's name or the certification body's technical reviewer's signature would be needed in an audit plan. Those seem like unnecessary details.
D and F seem like the clear choices here. An audit plan should outline the sequence and timing of the audit activities, as well as the names of the auditees and auditors involved.
