I think we're overthinking this. If no one is assigned to review and approve, then the risks and controls can't be in any state other than 'New'. Option D has got to be the right answer. *chuckles* Unless the system is programmed by a bunch of pranksters, that is.

Hmm, I'm not so sure. What if the system has some default behavior where it automatically puts them in the 'In Review' or 'Awaiting Approval' state, even without assigned reviewers? I'm a bit torn between options A and D.

Option D makes the most sense to me. If there's no one assigned to approve the risk and control, then they shouldn't progress beyond the 'New' state. Simple as that!
