Hmm, I'm going with B) IOM firmware and D) SAS cabling. If the cables are messed up, that could definitely cause a disk/shelf count mismatch. Though I gotta say, this question is making me feel a bit 'SAS-sy' today!

A) drive paths? Really? That's a bit of a stretch. I'd say B) IOM firmware and C) drive firmware are the obvious choices here.
