D is tempting, but I'm not sure a 12-instance scale set is necessary. Availability Sets seem more appropriate for this scenario.

I think the answer is B. An Availability Set with three fault domains and one update domain sounds like the best way to ensure that App1 runs on at least eight virtual machines during planned maintenance.
