I think the answer is B and D. The spare disk /dev/sda1 is being used to restore full redundancy, and the version 1.2 metadata is for legacy bootloaders.

I'm pretty sure the answer is A and E. A failed disk and a spare disk being used to restore redundancy, and the [3/2] status showing the number of disks in the array and spares.
