Analyzing facility operating reports might give you a good data-driven perspective, but I feel like actually seeing the operations in person would be more convincing.

I would go with reviewing the safety violation records filed by production employees. They're the ones on the frontlines and likely to spot potential issues.
Observation of the facility during operations seems like the most comprehensive approach to identify safety hazards. You can directly see the workflows and potential issues.
