I'm going with option C as the false statement. APs have to be on the same channel for roaming to work properly.

Option A seems true to me. APs can use different channels, that's what allows the STA to roam seamlessly.

I'm pretty sure option D is the correct answer. The STA sends a Reassociation Request frame to the foreign AP during roaming.
