I think option B is a trick question. Storing IIHI on a smart card would actually be a way of protecting the patient's privacy, not the other way around.
I'm not sure about that. Isn't IIHI supposed to be information that's directly related to a patient's health status or care? Option C about a credit company doesn't sound like it would qualify.
Option D seems like the right answer to me. A health care clearinghouse is responsible for processing claims, so any information they create or receive would be considered IIHI.
I'm not sure about this one, but I think option A could also be a possibility. Transmitting information to a business associate for payment purposes only could also make it individually identifiable.
I see your point, but I still think option D is the best choice. Health care clearinghouses are specifically involved in processing claims, so the information they handle would definitely be considered IIHI.
