Ah, I was leaning towards B) /var/netscaler, but that's not the right answer. Gotta remember that the login forms go in the /flash/nsconfig/loginschema folder.

The correct answer is C) /flash/nsconfig/loginschema. This is the standard location where custom login forms are uploaded on the Citrix ADC.
