Hmm, I'm leaning towards B. An AP cannot connect to the default gateway. If the AP can't reach the network, it might revert to the default SSID.

I'm going with D. An AP has Site Survey mode enabled. That would explain why the default SSID is visible, as the AP is in a mode to help with deployment.
