I agree with Leonida. Option E is completely irrelevant to the problem at hand. M needs to focus on the production process, not the employees' credentials.

I'm not sure that option B is the best solution. Having staff sign a form doesn't actually address the root cause of the defects. It's just a paperwork exercise.

Option D seems like the most reasonable choice here. Regular servicing of the machinery as recommended by the supplier is a good way to ensure product quality and reduce defects.
