Ooh, this is a tricky one. I'm tempted to say 'A fault', but I think 'A defect' is the more widely accepted term in the industry. Time to dust off those terminology skills!
This question is really testing our terminology knowledge, isn't it? I'm going to go with 'A defect' since that's the term I've heard used most often in software development.
