Dude, this is a gimme. It's totally the Write to Data Store Entity node. That's where the magic happens. Unless, of course, you're a wizard and can just wave your wand and make the data appear. In that case, the other options might work too.

I'm pretty sure it's the User Input Task that's the critical step. That's where you define the inputs that will get passed back to the process. Gotta get that right, or the whole thing falls apart.

The Data Management tab is definitely important, but I think configuring the Write to Data Store Entity node is the key step here. That's where you really connect the form data back to the process model.
