Personally, I think option C is a bit too narrow. Risk identification should be about understanding the risks, not just determining the responses to them.
Haha, option D is just asking for trouble. Identifying past risks is helpful, but we need to focus on what might happen in the future, not what already happened!
Option B seems the most appropriate. The main purpose of risk identification is to identify and describe all the potential risks that might occur on the project.
