D has got to be the right answer. Why would the form extensions be anywhere else but the forms directory? Easy peasy!

Yeah, D makes the most sense. It's where you would expect to find form-related functionality.

Hmm, this is a tricky one. I'm going to go with C, activiti-app/editor/index.html. The editor seems like the most likely place for form extensions to be registered.
